The Primary Care Program at the Veterans Affairs Medical Center New York

The Primary Care Program at the VA Medical Center New York consists of four interdisciplinary practice teams-two general medicine teams, one geriatric team, and one mental health team-each supported by on-site nursing, social work, mental health, nutrition, and pharmacy staff.

The program has an enrollment of approximately 17,000 active patients with a broad range of chronic illnesses, and provides some 46,000 continuity visits per year. It also offers special clinical services for anticoagulation, diabetes, and lipid management.
