Personal Goals Statement, emphasizing your professional goals in the field of infectious diseases, rather than your reasons for choosing infectious diseases as a subspecialty
3 letters of reference, including one from the Program Director or Department Chair of your present program
USMLE/COMLEX transcript
Our deadline for applications is February 15, 2009, and we expect to schedule interviews from late February through the end of April.
