Staff who work with radiation sources are usually required to wear personal radiation monitor badges to monitor their exposure. These badges are issued from the main office, or through the satellite offices at other locations. An application form must be completed and sent by FAX or internal mail to the Radiation Safety Office. The Radiation Safety Officer (RSO) will determine if monitoring is necessary and what type of monitoring is appropriate. Badges must be returned for analysis periodically. Replacement badges are distributed to personnel through a coordinator in each department, who is responsible for timely badge exchange. Reports of exposure are reviewed by the RSO as they are received and individuals are advised in writing or by telephone of significant or unusual exposures. Written reports of exposure are supplied annually to each individual who receives a badge. The main office maintains the archive of badge records and will supply exposure records to personnel and employers upon written request.
